• Rapid weight loss
• Bed sores
• Sudden/unexpected emotional instability
• Unexplained falls
• Unexplained or unexpected death
• Unexplained injuries
• Broken bones
• Sedation/Medication used in place of supervision
• Resident incontinent and left in urine or feces
• Injury resulting from use or restraint or lack of restraint
